This template can be used for tracking licensure hours.  Review and edit according to discipline (ie LAMFTs have to track couple/family hours separate to ensure 1000 direct couple/family hours).  Supervisees can add tabs for each month.  It is highly encouraged to use a separate spreadsheet for each setting/supervisor if the supervisee is tracking hours in multiple settings or under multiple supervisors.
